I am on the city's group health insurance, how do I get reimbursed for the outpatient / inpatient surgery co-pays?
Your health insurance vendor may send an Explanation of Benefits (EOB) to your home each time they receive a claim on your behalf that is subject to an eligible co-pay. If you don't receive it within two months of service, please contact your health insurance provider and request the EOB. When you receive the EOB that shows your inpatient or outpatient surgery co-pay, bring that EOB into Human Resources to request a reimbursement.
